Job Description:
  • The Care Coordinator for Harm Reduction will work at Fair Haven Community Health Care's shelter- and street-based services.
  • This role will coordinate client intakes and non-clinical services while addressing social and structural factors that impact clients' health.
  • The ideal candidate will have a recovery-oriented approach and uphold the principles of harm reduction in their work.
Key Responsibilities:
  • Liaise with organizations and care providers to link clients to available services.
  • Develop and evaluate shared plans of care for patients.
  • Link patients with social needs to community resources.
  • Assist with and follow-up on health maintenance items and chronic disease management.
  • Conduct home visits as needed.
  • Identify barriers to care impacting clients' treatment adherence.
  • Collaborate with clinical teams to meet the clinical and social needs of high-needs patients.
