Job Summary

The Clinical Pharmacist II plays a vital role in supporting the vision, mission, and goals of Yale New Haven Health and the Department of Pharmacy.

Main Responsibilities
  • Customer Service Quality
    • Demonstrate and promote CHAMP behaviors to ensure exceptional patient care.
  • Medication Management
    • Conduct timely and accurate medication order reviews, considering patient characteristics, laboratory values, and vital signs.
  • Medication Preparation and Distribution
    • Demonstrate proficiency in pharmacy automation and technology as applicable to the work area.
  • Medication Safety, Regulatory Affairs, and Quality Assurance
    • Adhere to all departmental policies, procedures, protocols, and guidelines to ensure a safe and efficient environment.
  • Leadership
    • Provide education and training to pharmacy staff members, students, residents, and healthcare team members through inservice or preceptorship.
  • Financial/Volume Results
    • Contribute to departmental cost-reduction and revenue generation initiatives.
Qualifications
  • Education
    • Graduation from an accredited College of Pharmacy with a B.S. or PharmD degree.
  • Experience
    • Minimum of two years of relevant experience specific to the practice area.
  • Licensure
    • Current Connecticut Pharmacist License or eligibility for licensure in the state of Connecticut.
  • Special Skills
    • Demonstrate in-depth knowledge of pharmacology and therapeutics, as well as understanding of pharmacy systems of drug distribution and formulary management.
    • Excellent verbal and written communication skills and the ability to work collaboratively with others.
