Patient Care Coordinator

We are seeking a highly skilled and compassionate Patient Care Coordinator to join our team at NEXT CARE HOSPICE. As a key member of our care team, you will play a vital role in ensuring the efficient and effective scheduling of hospice and palliative care services.
Key Responsibilities- Answer phone calls and assist hospice patients and their families with scheduling appointments, refills, and other needs.
- Manage and maintain the calendars of LVNs, RNs, CHHAs, and on-call staff.
- Coordinate with medical staff to schedule continuous care and PRN visits as needed.
- Input and update patient information in the electronic medical record system.
- Provide exceptional customer service to patients and address any inquiries or concerns.
- Collaborate with other departments to coordinate patient care and scheduling needs.
- Order hard and soft DME, maintain supply inventory, and make cost-saving improvements.
- Complete prior authorizations, verify benefits, and prepare charts for billing to ensure timely reimbursement.
- Previous hospice experience required (minimum one year).
- Bilingual required.
- Familiarity with electronic medical record systems is a plus.
- Strong communication skills, both verbal and written.
- Excellent organizational skills and attention to detail.
- Ability to multitask and prioritize tasks effectively.
- Proficient in computer skills, including Google Suite.
