Role Responsibilities:
- Communicates with primary care providers to notify them of patient discharges via fax.
- Contacts primary care providers to secure necessary orders.
- Ensures that all new patient admissions are processed daily and appropriately assigned.
- Completes daily recertifications, assessing the need for recertification at the end of each episode, and coordinates staffing with support staff.
- Manages and responds to all electronic communications.
- Requests necessary authorizations for patient care.
- Possesses knowledge of DSL and the methods to access patient information, including primary care providers and orders.
- Oversees the completion of laboratory tests, ensuring results are documented in patient charts and communicated to the ordering physician, primary care provider, and pharmacy when required.
- Schedules and documents visits as needed, particularly when support staff is unavailable.
- Verifies that all laboratory forms are accurately completed.
- Confirms information with hospitals and updates quality assurance records.
- Ensures that all required documentation is completed and orders are updated in patient charts.
- Records communication notes regarding any changes and documents all phone interactions.
- Manages care plans, fall reports, and incident reports to ensure timely completion.
- Generates orders for other healthcare professionals as needed.
- Contacts primary care providers when there are changes in patient conditions.
- Understands the workflow processes within the organization.
- Demonstrates a comprehensive understanding of various insurance plans.
- Submits reports for Regal cases on a bi-weekly basis.
- Participates in case management meetings.
- Completes any additional tasks as assigned by management.
Qualifications:
Must hold a valid LVN license or possess over two years of relevant experience in the healthcare field.
