We are seeking a skilled RN Case Manager to join our team at the University of California, Irvine. As a key member of our healthcare team, you will play a vital role in ensuring the delivery of high-quality patient care.
Key Responsibilities- Assess patient treatment needs and develop, monitor, and evaluate treatment plans and progress.
- Facilitate interdisciplinary approaches to patient care, coordinating with healthcare professionals to ensure safe, timely, effective, efficient, and patient-centered care.
- Develop and maintain effective working relationships across the health system to ensure seamless care coordination.
- Current California RN license.
- Three to five years of experience in an acute care hospital setting, with a preference for medical/surgical experience.
- Previous experience as a Case Manager, with a strong understanding of Medicare, Medi-Cal, and insurance guidelines.
- Excellent written and verbal communication skills in English.
- Ability to maintain a work pace appropriate to the workload and establish effective working relationships across the health system.
- Broad knowledge of University and medical center organizations, policies, procedures, and forms.
- Critical care experience, particularly for Resource Case Managers.
