We are looking for a dedicated Clinical Intake and Patient Care Specialist to manage the daily intake operations for our Hospice services at NEXT CARE HOSPICE. Candidates with LVN or RN licenses are preferred. Experience in Hospice intake and proficiency in Spanish are highly valued.
Key Responsibilities:- Execute duties assigned by the Director of Nursing efficiently.
- Support the patient intake process and coordinate care effectively.
- Establish and maintain strong relationships with Case Managers, Nursing Facilities, Hospitals, Physicians, and other referral sources.
- Assist in organizing ambulance transport, procuring necessary supplies and equipment, and conducting bedside meetings.
- Engage with hospitals, foundations, nursing homes, and provide clear explanations of Hospice Benefits to patients and their families.
- Facilitate both in-person and telephonic appointments with patients' families and healthcare providers.
- Must possess a valid LVN or RN license.
- Prior experience in healthcare administration or related fields is essential; Hospice experience is preferred.
- Understanding of the patient intake process is required.
- Ability to collaborate with a diverse range of healthcare professionals and administrators.
- Exceptional organizational skills are necessary.
- Strong communication and follow-up abilities are essential.
