Position Overview
Type: Per Diem

The Clinical Care Coordinator is responsible for managing and facilitating the clinical oversight for a designated patient population. This role involves developing, monitoring, and enforcing care standards and protocols aimed at enhancing quality while promoting cost-effective use of healthcare resources.

Key Responsibilities
1. Collaborates with patients, families, caregivers, and the healthcare team to create a comprehensive care plan. This includes early identification and thorough assessment of patient needs/preferences and available resources. Determines the suitable level of care and coordinates the interdisciplinary treatment and care plan.
2. Ensures the advancement of care by implementing the care plan to achieve desired outcomes. Monitors progress towards goals and alerts the healthcare team to necessary adjustments in response to changes in patient needs and conditions.
3. Manages incoming calls and addresses inquiries from patients, families, and other healthcare team members within the scope of a registered nurse's practice.
4. Communicates vital information to the healthcare team and those involved in the patient's treatment and care.
5. Advocates for patients, families, and caregivers to ensure access to services and insurance benefits, while adhering to regulations and safeguarding patient health, safety, and rights.
6. Identifies situations that necessitate referrals to Quality, Risk Management, or Patient and Family Relations and discusses these with leadership.
7. Evaluates available options to balance cost and quality, ensuring optimal clinical and financial outcomes while facilitating progression to appropriate levels of care.
8. Assesses patient outcomes influenced by the care management process for opportunities for improvement. Engages in quality improvement initiatives within the clinical setting or other organizational committees.
9. Collaborates with clinical research teams to coordinate care for patients involved in complex protocols.
10. Performs additional duties as assigned.

Qualifications
  • Graduation from an accredited Nursing Program is required.
  • A Bachelor's degree (or enrollment in a Bachelor's degree program with completion within an agreed timeframe) along with 3 years of clinical experience is required.
  • Previous experience in Case Management is essential.
  • Recent experience in an acute care hospital setting is preferred.
  • Desired attributes include: collaborative team player with strong communication skills, a systems approach to planning, problem-solving, and decision-making, creativity, innovation, risk-taking, autonomy, flexibility, openness to change, and a commitment to professional development.
  • Proficiency in computer skills is required.
Licensure/Certifications Required

- Current Licensed Registered Nurse in New Hampshire is required.
- Basic Life Support (BLS) Certification is required.
