Position Summary:
A dedicated SOCIAL WORKER with a focus on providing compassionate care and support to patients.

This role involves delivering assessments, guidance, information, advocacy services, and additional resource assistance to patients, either directly or through consultation. Experience in social work, along with innovative problem-solving skills, is essential.

Key Responsibilities:

  • Perform comprehensive psychosocial assessments for patients and their families.
  • Provide counseling services to individuals and families facing social and emotional difficulties.
  • Coordinate community resources to address identified needs effectively.
  • Support families in navigating complex care requirements.
  • Offer consultation, assessment, referral, and coordination in cases of potential abuse or neglect.
  • Accurately document clinical information in medical records.
  • Complete assigned duties as required.

Required Qualifications:

  • Master's degree in Social Work is mandatory.
  • Previous experience in social work is preferred.
  • Exceptional organizational, communication, decision-making, and time management skills are required.
  • Proficiency in negotiation and conflict resolution is necessary.
  • Basic computer literacy is essential.
