Substance Abuse Case Coordinator

This role is focused on delivering comprehensive management services for individuals dealing with substance use challenges. It encompasses proactive connections to more advanced substance use disorder treatments, infectious disease care, and various social services, including vocational training and insurance acquisition. The case manager will primarily operate within a clinical setting but will also engage with the community to ensure clients remain connected to essential services. Additionally, this position involves providing brief counseling aligned with regulations for office-based opioid treatment.


Key Responsibilities
  • Facilitate client access to services addressing substance use disorders, infectious disease prevention, and additional social service requirements.
  • Conduct monthly brief counseling sessions with clients, increasing frequency as necessary.
  • Organize check-in meetings with client support networks as needed.
  • Prepare and submit program reports as mandated.
  • Offer support services to enhance understanding of individual substance use issues.
  • Stay informed about treatment alternatives, medical care, patient assistance programs, housing, and employment support services.
  • Participate in relevant training and regular supervisory meetings.
  • Perform other assigned duties as required.
  • Maintain a detailed log of communications.

Qualifications
  • A Bachelor's degree in Social Work or a related discipline; or equivalent experience and training.
  • Exceptional communication skills, with the ability to engage effectively across all organizational and community levels.
  • Proficiency in Microsoft Office Suite, including Word, Excel, and PowerPoint.
  • Outstanding organizational and interpersonal skills.
  • Knowledge of and ability to navigate community resources.
