The Social Services Case Coordinator is responsible for the development, coordination, and delivery of comprehensive case management services to patients and their families. This role encompasses a variety of duties aimed at ensuring effective support and intervention.

Work Schedule: Monday - Friday, 8:00 AM - 4:30 PM

Key Responsibilities:

Deliver social services to patients and their families/significant others:

  • Conduct pre-admission consultations with community organizations, families, or patients.
  • Execute the social services component of the Multi-Disciplinary Assessment Tool within 24 hours of patient admission.
  • Secure necessary consents for information release in compliance with hospital policies.
  • Provide therapeutic services to individuals, groups, and families in accordance with program standards.
  • Facilitate educational sessions for patients, families, and significant others.
  • Implement goals, objectives, and interventions as outlined in the Master Treatment Plan within the therapeutic framework of the patient's program.
  • Utilize expertise to gather and analyze information regarding the patient's condition, ensuring age-appropriate interventions are provided.
  • Maintain follow-up communication with patients as dictated by program guidelines.
  • Work autonomously with minimal oversight.
Ensure timely completion of assignments and documentation.
  • Draft clear, concise, and achievable treatment goals in the treatment plan and weekly reviews for each issue addressed by Social Services.
  • Maintain ongoing treatment documentation in line with program requirements.
  • Document the patient's treatment status in the medical record, including progress notes.
  • Prepare discharge summaries that detail the patient's progress, presenting issues, current status, and recommendations for post-discharge care.
  • Collaborate with the case social worker on discharge planning.
  • Prepare necessary written reports as assigned.
  • Report and document any suspected cases of abuse or neglect in accordance with legal and hospital protocols.
Communicate with professionalism and accuracy.
  • Collaborate with other departments and agencies as needed, adhering to Social Services policies.
  • Address challenging issues promptly with patients, families, and colleagues, keeping the Social Services manager informed.
  • Seek supervision for problem resolution when necessary and actively participate in departmental meetings.
Exhibit professionalism.
  • Comply with the hospital's dress code and established standards of practice.
  • Adhere to all personnel and Social Services department policies and procedures.
  • Engage in mandatory training sessions and performance improvement initiatives as requested.
  • Report safety hazards to Environmental Services and the Social Services manager promptly.
  • Follow the hospital's safety and employee health programs.
Additional Qualifications:

Education and Training:
  • A Master's Degree in Social Work (MSW) or a related mental health field is required.
  • A minimum of three years of experience in a similar role within a healthcare or related environment.
  • Comprehensive knowledge of community resources, human behavior dynamics, and intervention techniques.
  • Commitment to ongoing education in relevant areas.
  • Current CPR certification from the American Heart Association or American Red Cross is required upon hire, with annual re-certification.
  • Formal training in managing aggressive patients is required within the first 60 days of employment, with annual re-certification.
  • Experience working in a clinical setting with children facing mental health challenges is required for specific programs.
