- Patient Education: Provide comprehensive education on HIV/HCV and sexually transmitted infections, as well as pre-exposure prophylaxis (PrEP) navigation and HIV/HCV counseling and testing.
- Client Support: Offer ongoing support and guidance to patients, including peer recovery support and regular check-in meetings.
- Program Reporting: Compile and submit program reports as required, ensuring accurate and timely data entry.
- Mobile Unit Operations: Conduct patient navigator duties on the mobile Syringe Services Mobile Unit as needed.
- Health Record Management: Enter data into the electronic health record and third-party payor systems.
- Education: Bachelor's degree in Social Work or related field; or equivalent experience and training.
- Skills: Excellent verbal and written communication skills, comprehensive knowledge of substance abuse treatment, and ability to interact effectively with staff, clients, and agencies.
- Experience: Demonstrated effective supervisory skills, ability to assess crises and respond appropriately, and ability to maintain client confidentiality in a culturally competent setting.
