Position Overview:

The Care Coordination Support Specialist plays a vital role in assisting with administrative care management functions. This includes engaging in outreach efforts, handling incoming calls, and organizing service schedules.

This is a remote opportunity.


Key Responsibilities:


1. Conduct outreach to members via telephone to facilitate care plan follow-ups, provide information on community or health plan resources, and address inquiries or concerns related to scheduling and ongoing education for both members and providers.

2. Assist members in connecting with additional health plan and community resources to guarantee high-quality customer service.

3. Serve as the primary support for various member and provider inquiries, which may involve clarifying care plan procedures and protocols.

4. Support member onboarding and manage daily administrative tasks, including sending welcome letters, relevant correspondence, and educational materials to foster a successful member/provider relationship.

5. Document and maintain non-clinical member records to ensure compliance with state and regulatory standards, providing necessary information to providers as required.

6. Demonstrate knowledge of local benefits and resources, making referrals to address Social Determinants of Health (SDOH) needs.

7. Perform additional duties as assigned.


Working Hours:

8:00 AM - 5:00 PM


Required Skills:

- Proficient in data entry and Microsoft Excel

- Familiarity with PowerPoint

- Strong communication skills

- Ability to summarize findings effectively

- Organized and detail-oriented

- Capacity to manage multiple tasks simultaneously


Qualifications:

- High School Diploma or GED required

- Experience in a related field (1-4 years preferred)

- Familiarity with medical records is a plus
