Job Title: Service Coordinator

We are seeking a highly motivated Service Coordinator to join our team at New Opportunities. As a Service Coordinator, you will be responsible for providing critical support services to individuals experiencing homelessness in the Middlesex, Meriden, and Wallingford areas.

Salary: $20.00 per hour

About the Role:

This is a full-time, non-union position that requires a high level of energy, flexibility, and adaptability. As a Service Coordinator, you will be working as part of the larger MMW CAN outreach team, engaging with consumers, assessing their needs, and connecting them with vital resources and services.

Your primary responsibilities will include:

* Performing CAN assessments to identify individual needs and develop personalized service plans
* Enrolling consumers in the H2H program and ensuring they are on the By Name List
* Developing Housing Services and Sustainability Plans to support long-term stability

Requirements:

To be successful in this role, you will need to possess the following skills and qualifications:

* A B.A. degree plus one year of experience in case management or an A.S. degree in Human Services plus three years of experience in community-based case management
* Familiarity with the CT Homelessness Management Information System (CT HMIS)
* Previous experience working with families and individuals experiencing homelessness
* Knowledge of various CT housing programs, including Rapid Rehousing, Shelter Plus Care, and Permanent Supportive Housing
