Central Ohio Primary Care is in search of a dedicated Clinical Practice Provider (CPP) to join our esteemed Hospitalists team. The Clinical Practice Provider will deliver comprehensive healthcare services to patients admitted to the hospital, adhering to a standard care agreement or supervisory arrangement with an attending physician from Central Ohio Primary Care (COPC) Hospitalists. This role involves collaboration with physicians and other healthcare professionals to ensure a complete spectrum of patient care within the hospital environment.

Key Responsibilities:

  • Conducting patient rounds in Observation Units.
  • Participating in a collaborative care model within an Accountable Care Unit alongside a Physician and a full support team.
  • Assisting in our Inpatient Support Center to address immediate issues concerning hospitalized patients.
  • Facilitating same-day pre-admission testing clearance.
  • Implementing Value-Based Care strategies for hospitalized patients.
  • Providing ongoing support during patient transitions in and out of the hospital.
  • Engaging in monthly educational meetings for APPs.
  • Receiving updates and educational resources on Hospitalist practices monthly.
Qualifications and Requirements:
  • Possession of a current certificate of authority to practice as an APRN in Ohio, along with a Master's degree in nursing from an accredited Nurse Practitioner program, qualifying for national certification.
  • Graduation from a PA-C program eligible for certification by the National Certification Commission for Physician Assistants (NCCPA).
  • Preferred experience in a hospital setting.
  • Demonstrated flexibility and a strong desire to learn.
