Job Summary
The Advanced Practice Provider (APP) is a key member of our healthcare team, responsible for providing high-quality patient care services in collaboration with licensed physicians. This role requires a strong foundation in nursing or medicine, with a focus on preventive health, acute care, and chronic condition management.
Key Responsibilities
- Provide comprehensive patient care services, including health promotion, disease prevention, and management of acute and chronic conditions.
- Collaborate with physicians and other healthcare professionals to ensure seamless patient care.
- Develop and implement individualized care plans, taking into account patient needs, preferences, and goals.
- Conduct thorough assessments, diagnose conditions, and develop treatment plans in consultation with physicians.
- Order and interpret diagnostic tests, prescribe medications, and perform procedures as authorized.
- Communicate effectively with patients, families, and healthcare teams to ensure informed decision-making and optimal patient outcomes.
Requirements
- Current certification as an APRN in the State of Ohio or eligibility for licensure as a PA-C.
- Master's degree in Nursing or a related field, or a graduate degree in a PA-C program.
- 1 year of relevant experience in a healthcare setting, preferably in an inpatient or acute care environment.
