Role Summary

The Advanced Practice Provider (APP) plays a crucial role in delivering direct patient care services within the Post-Acute Care environment.

Key Responsibilities

  • Conduct clinical assessments and follow-up visits
  • Evaluate and manage chronic health conditions
  • Assess and address changes in residents' health status
  • Review and manage medications, orders, and diagnostic evaluations
  • Collaborate with physicians to coordinate patient care
  • Perform Annual Wellness Visits and other essential evaluations
  • Work alongside interdisciplinary teams and maintain communication with families
  • Undertake additional responsibilities as assigned

Qualifications

  • Current nurse practitioner or physician assistant licensure
  • Master’s Degree in Nursing or Physician Assistant Studies
  • Strong leadership and interpersonal abilities
  • Excellent verbal and written communication skills
  • Organized, punctual, and a team player
