Job Overview

Position Purpose:

The primary aim of this role is to assist and advocate for kinship families by delivering and managing comprehensive case management services. This includes organizing support groups, training sessions, in-home therapeutic interventions, crisis response, and transitional assistance. The Kinship Connections Case Manager will handle all referrals and orchestrate services and admissions for designated clients.

Key Responsibilities:

  • Conduct ongoing support groups and training sessions.
  • Perform family and client assessments, crafting tailored service plans.
  • Facilitate goal-setting and resource coordination to aid families in transitions and behavior stabilization.
  • Accurately document all case information in the designated data management system, adhering to established timelines.
  • Travel within the service area while demonstrating cultural and socioeconomic sensitivity to the client population.

Qualifications:

Education: A master's degree in social work, psychology, or a related human services discipline from an accredited institution is preferred, along with a relevant state license. Alternatively, a bachelor's degree in the same fields is required, also accompanied by a state license.

Experience: The role necessitates either a graduate degree or a minimum of two years of cumulative experience with an undergraduate degree, specifically in crisis intervention, intake and assessment, and case management involving children and families. A solid understanding of crisis intervention and trauma-informed care practices is essential, along with proficient verbal and written communication skills and basic computer literacy.

Functional Expectations:

  • Deliver timely services to clients and families, ensuring regular home visits for those receiving in-home services.
  • Conduct ongoing assessments, service planning, resource development, risk evaluations, safety planning, and advocacy.
  • Facilitate and document support groups and training sessions as required.
  • Collaborate effectively within a multi-disciplinary team, maintaining accurate and timely documentation of services provided.
  • Be available for on-call emergencies related to assigned families and children.

Working Environment:

Engagement with children exhibiting behavioral challenges, including potential physical aggression, is part of the role. There may also be exposure to illness and crisis situations within families. A reliable personal vehicle is necessary for travel, and maintaining a good driving record is required.

Community Engagement:

  • Foster collaborative relationships with referral agencies and maintain positive interactions with families, community organizations, and service providers.
  • Ensure compliance with service delivery standards and documentation as per agreements and organizational policies.

Documentation and Reporting:

  • Maintain comprehensive and up-to-date case records.
  • Develop and document initial family plans and ongoing reviews.
  • Participate in scheduled supervision and case discussions.
