Position Overview:

The primary role of the Resource Navigator is to assist and empower youth and their families by delivering and coordinating access to essential services and navigating complex systems to address their specific needs. This position is crucial in ensuring timely responses to all referrals. The Resource Navigator conducts comprehensive assessments of strengths and needs, linking families to vital community resources while continuously enhancing their understanding of state and federal service programs. Sensitivity to the cultural and socioeconomic backgrounds of the service population is essential, along with effective collaboration with personnel from various organizations in a community-based setting.

Educational Qualifications:

A bachelor's degree in social work, psychology, or a related field from an accredited institution is preferred. Alternatively, one additional year of relevant experience in case management or service coordination may substitute for a year of college education (30 credit hours) required for a bachelor's degree. Bilingual proficiency is highly desirable.

Experience Requirements:

The candidate should possess at least one year of experience in case management or in facilitating connections for families within federal or state service navigation systems, such as WIC or TANF. A solid understanding of crisis intervention and trauma-informed care practices is necessary, along with strong verbal and written communication skills. Proficiency in basic computer skills is also required.

Key Responsibilities:

  • Conduct ongoing assessments and service navigation for assigned families.
  • Maintain accurate and timely documentation of services provided.
  • Prepare and submit all necessary reports and statistical data promptly.
  • Coordinate with referral agencies, family members, and other support systems to fulfill family needs.
  • Ensure compliance with HIPAA regulations while communicating with associated agencies.
  • Provide on-call support as needed.

Working Environment:

Exposure to children and youth with behavioral challenges, including potential physical aggression, as well as exposure to health-related risks. The position requires working in shared spaces with other agencies, necessitating respect for their policies and procedures.

  • Collaborate with community organizations to share resources and information.
  • Represent the organization positively and professionally.
  • Stay informed about community issues and priorities.

Documentation and Reporting:

  • Maintain up-to-date case records and document assessments.
  • Provide regular case management services for each participant.
  • Complete discharge and aftercare plans for all program participants.
