Youth Services Case Coordinator

Position Purpose: The role is designed to assist and empower young individuals and their families by delivering and managing comprehensive case management services, therapeutic interventions, crisis response, and transitional support.
Key Responsibilities:
- Respond promptly to all client referrals and manage admissions for designated programs.
- Conduct admission evaluations, formulate service plans, and aid in establishing objectives while coordinating resources to enhance behavioral stability and promote independent living skills for youth moving towards permanency or self-sufficiency.
- Maintain a deep understanding of the cultural and socioeconomic backgrounds of the service population.
Qualifications:
Education: A Bachelor’s degree in social work, psychology, or a related field from an accredited institution is mandatory. A Master’s degree and/or additional certifications are advantageous.
Experience: A minimum of two years of relevant experience is required, particularly in working with youth, crisis intervention, intake assessments, and case management. Proficiency in crisis intervention techniques, trauma-informed care, and effective counseling methods for adolescents and families is essential. Strong verbal and written communication skills, along with basic computer proficiency, are necessary.
Functional Expectations:
- Deliver ongoing assessments, service planning, and case management for all assigned clients.
- Ensure accurate and timely documentation of services rendered.
- Prepare and submit all required reports and statistical data punctually.
- Collaborate with referral agencies, family members, and other support systems to address the needs of youth effectively.
- Communicate efficiently with campus staff to align service plans for each youth and facilitate group activities focusing on life skills and team building.
Work Environment:
Exposure to children exhibiting behavioral challenges, including potential physical aggression, and possible exposure to health-related risks.
Confidentiality:
Uphold confidentiality and adhere to policies concerning personal and client records.
Community Engagement:
- Foster collaborative relationships with referral agencies and maintain positive interactions with local youth and human services organizations.
- Represent the organization in a professional manner.
Documentation Responsibilities:
- Maintain current case records and document initial assessments and service plans.
- Provide regular updates on case progress and communicate client needs to the direct care team.
- Assist in the development of program-related documentation and evaluation tools.
Workplace Conduct:
- Adhere to organizational policies and procedures consistently.
- Maintain a positive work environment and advocate for youth and families.
- Collaborate effectively with colleagues to foster a productive atmosphere.
Additional Duties:
- Participate in team meetings and community training sessions as required.
- Complete necessary training and certifications in line with licensing standards.
- Assist with other organizational activities as assigned.
