JOB TITLE: Registered Nurse Case Manager

REPORTS TO: Director of Clinical Services
POSITION SUMMARY:
The Registered Nurse is responsible for planning, organizing, and overseeing hospice care, drawing on extensive nursing experience with a focus on community health education. This professional role utilizes community resources to tailor services that address the unique needs of patients and their families within their home environments.
KEY RESPONSIBILITIES:
- Conduct initial, comprehensive, and ongoing assessments of patients and families to identify hospice needs, ensuring thorough physical evaluations and histories of current and past illnesses.
- Deliver professional nursing care by applying all aspects of the nursing process and leveraging health assessment data to formulate nursing diagnoses.
- Regularly assess and evaluate patient status, documenting and adjusting care plans as necessary to meet evolving patient and family needs.
- Develop individualized care plans that set achievable goals based on nursing diagnoses, actively involving patients and families in the planning process.
- Implement preventive and rehabilitative nursing procedures, administering medications and treatments as outlined in the physician's care plan.
- Assist patients with daily living activities, promoting self-sufficiency and optimal comfort care.
- Serve as Case Manager, coordinating comprehensive patient care for an assigned caseload.
- Maintain accurate clinical documentation regarding patient conditions and care provided, including pain and symptom management updates.
- Communicate effectively with physicians regarding patient needs and condition changes, ensuring timely receipt of orders as required.
- Educate patients and families on self-care techniques, providing guidance on medications, dietary needs, and recognizing opportunities for health counseling.
- Collaborate with families and the hospice interdisciplinary team to address the emotional needs of patients and their caregivers.
- Participate in on-call duties as outlined in the on-call policy.
- Engage in quality assessment and performance improvement initiatives.
- Perform additional responsibilities as assigned.
QUALIFICATIONS:
A Bachelor's or Associate's degree in nursing or a related field is required; a Master's degree in Nursing or a related healthcare field is preferred. Candidates must hold an active nursing license in good standing and maintain current CPR certification. A minimum of 2-5 years of experience in healthcare management, hospice, or palliative care is preferred, along with at least 1 year of experience with Medicare Hospice Certification and CHAP guidelines. Knowledge of hospice care and interdisciplinary services is essential. Proficiency in MS Office applications and the ability to learn job-specific software systems are required. Strong organizational, verbal, and written communication skills, along with analytical problem-solving abilities and attention to detail, are critical.
