POSITION SUMMARY:


The hospice aide serves as a vital member of the interdisciplinary team, working under the guidance of a registered nurse to provide essential services that cater to the personal needs of patients, ensuring their comfort and dignity.

The hospice aide is tasked with monitoring the patient's condition, documenting care provided, and reporting observations to the appropriate personnel. Assignments are structured to enhance the quality, continuity, and safety of patient care.

KEY RESPONSIBILITIES:
1. Deliver personal care services based on assigned patient needs, responding promptly to requests.
2. Maintain effective communication with patients and families, ensuring ethical and respectful interactions while upholding confidentiality.

3. Address safety requirements and utilize equipment correctly (e.g., foot stools, side rails, oxygen). Provide personal care such as bathing, back rubs, oral hygiene, and changing bed linens as required.

4. Assist patients with dressing and undressing as needed.
5. Aid in feeding patients as directed.

6. Measure and document vital signs, including temperature, pulse, respiration, and blood pressure, following established competencies.

7. Monitor and care for the patient's skin to prevent pressure sores, while also being aware of the emotional and interpersonal dynamics involving caregivers and family members.
8. Support patients in mobility and exercises as instructed by healthcare professionals.

9. Facilitate self-administered medications in accordance with state regulations, providing reminders as necessary.

10. Assist with light laundry tasks as assigned.
11. Conduct range of motion exercises and other basic procedures as part of therapy services, adhering to competency standards.

12. Offer respite care for patients and their families when present on-site.

13. Ensure the patient's living space is clean and organized, as per assignments.
14. Follow organizational protocols for documentation and care standards, maintaining professionalism at all times.
15. Participate in quality improvement initiatives and perform additional duties as required.

QUALIFICATIONS:
1. Maintain an active hospice aide certification in accordance with state regulations.
2. Preferably possess 1-3 years of experience in hospice or palliative care, with training in end-of-life support.
3. Have a solid understanding of hospice care and the interdisciplinary services provided to patients and their families.
4. Proficient in MS Office applications and capable of learning specific software systems related to the role.
5. Exhibit strong organizational skills and effective verbal and written communication abilities.
6. Demonstrate analytical skills for problem-solving and maintain a high level of attention to detail and accuracy.
