POSITION SUMMARY:

The hospice aide serves as a vital member of the interdisciplinary team, working under the guidance of a registered nurse to provide essential services tailored to meet the personal needs of patients and enhance their comfort. This role involves careful observation of the patient, timely reporting of findings, and thorough documentation of care provided. The hospice aide is assigned in a manner that ensures quality, continuity, and safety in patient care.

KEY RESPONSIBILITIES:
  1. Deliver personal care based on assigned patient needs, responding promptly to requests. If a task cannot be performed, communicate with the Case Manager immediately.
  2. Utilize effective communication techniques with patients and families, maintaining ethical standards and confidentiality.
  3. Ensure patient safety and proper use of equipment (e.g., foot stools, side rails, oxygen).
  4. Administer personal care tasks such as bathing, back rubs, oral hygiene, and changing bed linens as required.
  5. Assist patients with dressing and undressing as necessary.
  6. Support patients during mealtimes as assigned.
  7. Measure and document vital signs including temperatures, pulse, respiration, and blood pressure as directed, demonstrating competency in these skills.
  8. Monitor and care for the patient's skin to prevent tissue breakdown.
  9. Report significant changes in the patient's condition to the Case Manager, while being mindful of the caregiver dynamics.
  10. Facilitate ambulation and exercise as instructed by the hospice nurse or therapist.
  11. Provide range of motion exercises and assist with patient positioning as directed.
  12. Assist with self-administered medications as permitted by state regulations, offering reminders as needed.
  13. Support patients with bedpans and urinals, and assist with light laundry tasks as assigned.
  14. Perform simple procedures as part of therapy services as ordered, demonstrating required competencies.
  15. Offer respite care for patients and their families when present on-site.
  16. Maintain cleanliness and organization in the patient's living area as assigned.
  17. Adhere to the organization's standards for documentation and professional conduct.
  18. Engage in quality assessment and performance improvement initiatives.
  19. Fulfill additional responsibilities as assigned.
QUALIFICATIONS:

Possess an active hospice aide certification in accordance with state regulations. A minimum of 1-3 years of experience in hospice or palliative care, or training in end-of-life care is preferred. Knowledge of hospice services and interdisciplinary care is essential. Proficiency in MS Office applications is preferred, along with the ability to learn job-specific software. Strong organizational, verbal, and written communication skills are required, as well as analytical skills for effective problem-solving. Attention to detail and accuracy are critical.

LIMITATIONS:

The hospice aide will not engage in activities considered nursing practices as defined by the Nurse Practice Act. Specifically, the hospice aide will not administer medications, take physician orders, or perform procedures requiring the expertise of a licensed nurse.
