POSITION SUMMARY:

The Registered Nurse is responsible for planning, organizing, and directing hospice services, with a strong focus on community health education and experience. This professional role utilizes community resources to tailor services that address the unique needs of patients and their families in their own environments.

KEY RESPONSIBILITIES:

  1. Conducts initial, comprehensive, and ongoing assessments of patients and families to identify hospice requirements, including a thorough physical assessment and medical history.
  2. Delivers professional nursing care by applying all components of the nursing process and utilizing health assessment data to formulate nursing diagnoses.
  3. Regularly evaluates patient status, revising care plans as necessary based on patient and family needs.
  4. Creates individualized care plans that set achievable goals, informed by nursing diagnoses, and incorporates palliative care strategies, involving patients and families in the planning process.
  5. Implements preventive and rehabilitative nursing procedures, administering medications and treatments as outlined in the physician's care plan.
  6. Supports patients with daily living activities, promoting self-sufficiency and optimal comfort care.
  7. Acts as the Case Manager, overseeing patient care for an assigned caseload.
  8. Maintains accurate clinical documentation regarding patient conditions and care provided, including pain and symptom management outcomes.
  9. Communicates effectively with physicians regarding patient needs and condition changes, ensuring timely updates and order management.
  10. Educates patients and families on self-care techniques, medication management, dietary instructions, and health counseling opportunities.
  11. Collaborates with families and the hospice interdisciplinary team to address the emotional needs of patients and caregivers.
  12. Participates in on-call responsibilities as defined by the organization’s policy.
  13. Engages in quality assessment and performance improvement initiatives.
  14. Performs additional duties as assigned.

QUALIFICATIONS:

A Bachelor's or Associate's degree in nursing or a related field is required. A Master's degree in Nursing or a related healthcare discipline is preferred. An active nursing license in good standing is mandatory, along with current CPR certification. A minimum of 2-5 years of experience in healthcare management, hospice, or palliative care is preferred, with at least 1 year of experience in Medicare Hospice Certification and CHAP guidelines. Knowledge of hospice care and interdisciplinary service delivery is essential. Proficiency in MS Office applications and the ability to learn job-specific software systems are required. Strong organizational, verbal, and written communication skills, along with analytical problem-solving abilities and attention to detail, are critical.
