POSITION SUMMARY:
The Registered Nurse is responsible for planning, organizing, and directing hospice services, leveraging their expertise in nursing with a focus on community health education and experience. This role involves utilizing community resources to design and implement services tailored to the needs of patients and their families within their homes and communities.
KEY RESPONSIBILITIES:
- Conducts comprehensive assessments of patients and families to identify hospice needs, including thorough physical evaluations and medical histories.
- Delivers professional nursing care by applying all components of the nursing process and utilizing health assessment data to formulate nursing diagnoses.
- Monitors and evaluates patient status through the development and adjustment of care plans, regularly reassessing patient and family needs.
- Creates individualized care plans that set achievable goals based on nursing diagnoses, incorporating palliative care strategies while involving patients and families in the planning process.
- Implements necessary preventive and rehabilitative nursing actions, administering medications and treatments as outlined in the physician's care plan.
- Assists patients with daily living activities, promoting self-sufficiency and optimal comfort care.
- Serves as Case Manager, coordinating care for an assigned patient caseload.
- Maintains accurate clinical documentation regarding patient conditions and care provided, including pain and symptom management outcomes.
- Communicates effectively with physicians about patient needs and condition changes, ensuring timely receipt of physician orders.
- Educates patients and families on self-care techniques, providing instructions on medications, diet, and other health-related matters while seizing opportunities for health counseling.
- Collaborates with families and the hospice interdisciplinary team to address the emotional needs of patients and their caregivers.
- Participates in on-call responsibilities as defined by organizational policies.
- Engages in quality assessment and performance improvement initiatives.
- Performs additional responsibilities as assigned.
QUALIFICATIONS:
A Bachelor's or Associate's degree in nursing or a related field is required; a Master's degree in Nursing or a related healthcare discipline is preferred. An active nursing license in good standing is mandatory, along with current CPR certification. A minimum of 2-5 years of experience in healthcare management, hospice, or palliative care is preferred, including at least 1 year of experience with Medicare Hospice Certification and CHAP guidelines. Knowledge of hospice care and interdisciplinary services is essential. Proficiency in MS Office applications and the ability to learn specific software systems is required. Strong organizational, verbal, and written communication skills are necessary, along with analytical problem-solving abilities and meticulous attention to detail.
