SUMMARY:
The hospice aide serves as a vital member of the interdisciplinary team, operating under the guidance of a registered nurse. This role involves delivering essential services to patients, ensuring their personal needs are met while fostering comfort and dignity. The hospice aide is tasked with monitoring patient conditions, documenting care provided, and communicating observations effectively.
KEY RESPONSIBILITIES:
- Addressing personal care requirements based on assigned patients and responding to their needs promptly. Report any inability to perform specific tasks to the Case Manager without delay.
- Utilizing effective communication techniques to engage with patients and families, ensuring ethical and respectful interactions while maintaining confidentiality.
- Ensuring patient safety by using equipment correctly and adhering to safety protocols.
- Providing personal care services such as bathing, oral hygiene, and changing bed linens as required.
- Assisting patients with dressing and undressing as needed.
- Supporting patients during meals as assigned.
- Measuring and recording vital signs, including temperature, pulse, respiration, and blood pressure, as instructed.
- Monitoring skin condition to prevent tissue breakdown and reporting significant changes to the Case Manager.
- Assisting with mobility and exercise as directed by healthcare professionals.
- Facilitating normal range of motion exercises and proper positioning as instructed.
- Providing reminders for self-administered medications in accordance with state regulations.
- Offering assistance with bedpans and urinals, and helping with light laundry tasks as assigned.
- Conducting simple procedures as part of therapy services when appropriately trained.
- Providing respite care for patients and their families when present.
- Maintaining a clean and organized living space for patients as assigned.
- Adhering to documentation standards and maintaining professional conduct at all times.
- Participating in quality improvement initiatives and activities.
- Completing additional responsibilities as assigned.
QUALIFICATIONS:
Must hold an active hospice aide certification in accordance with state regulations. A minimum of 1-3 years of experience in hospice or palliative care is preferred, along with training in end-of-life care. Familiarity with hospice services and interdisciplinary team dynamics is essential. Proficiency in MS Office applications is preferred, along with the ability to learn specific software systems. Strong organizational, verbal, and written communication skills are necessary, along with attention to detail and accuracy.

This role requires frequent sitting and the use of hands for various tasks. Occasional standing and walking are necessary, along with the ability to lift and move up to 50 pounds.
LIMITATIONS:
The hospice aide will not engage in activities considered nursing practices as defined by the Nurse Practice Act, including administering medications or performing procedures requiring nursing expertise.
