Role Summary
The Registered Nurse plays a vital role in delivering compassionate care to elderly and terminally ill patients. This position involves assessing patient needs, developing individualized care plans, and providing essential end-of-life support to both patients and their families. Collaboration with a dedicated Care Team is crucial, requiring regular communication and teamwork.
Key Responsibilities:
- Conduct thorough assessments of the nursing requirements for Hospice patients.
- Offer nutritional guidance and support.
- Administer Hospice nursing services, treatments, and preventive care.
- Implement nursing procedures tailored to the patient's Hospice care and safety.
- Monitor and report any significant changes in the patient's physical or emotional state to the physician and interdisciplinary team.
- Educate and support patients and their families in providing care.
- Oversee and mentor other nursing staff.
- Collaborate with the interdisciplinary group to develop and update patient/family care plans.
- Execute specific nursing tasks as per physician directives.
- Participate in team meetings and discussions.
- Adhere to the policies and procedures of The Care Team, ensuring confidentiality and compliance with HIPAA regulations.
- Communicate promptly with supervisors regarding any unmet patient needs or procedural challenges.
- Engage in the on-call system, providing coverage as needed.
- Maintain and enhance nursing skills and knowledge.
- Coordinate care plans for patients in various healthcare settings.
- Manage time effectively to attend all mandatory meetings.
- Follow infection control protocols as per agency guidelines.
- Assist in training and orientation as required.
- Document all services provided in the clinical record within the designated timeframe.
- Perform additional duties as assigned by the Regional Branch Director.
Experience and Skills:
- Support strategic initiatives while optimizing operational efficiency.
- Uphold the agency's mission, philosophy, and core values.
- Ensure adherence to agency policies regarding patient care and safety.
- Maintain strict patient confidentiality in compliance with HIPAA regulations.
- Must be licensed or registered in the state of practice.
- Pass a criminal background check and motor vehicle record check.
- Submit a health statement confirming the ability to perform job duties.
- Possess a valid state driver's license and current automobile insurance.
- Hold a valid CPR certification; online certification accepted, hands-on preferred.
- Complete TB testing as per agency policy.
- Ability to work autonomously and make informed decisions swiftly.
- Capability to supervise others effectively and respond to crises outside of a hospital environment.
- Demonstrate adaptability to diverse social, cultural, and religious contexts.
- Completion of a Hospice training program is preferred.
- Active patient interaction within the last three years is preferred.
- A minimum of two years of experience as a registered nurse is preferred.
- Graduate from an accredited nursing program.
- Currently licensed as a registered nurse in the state of practice.
