ROLE SUMMARY:

The Outreach Specialist will be an integral part of a Street-based Multidisciplinary Team (MDT), focusing on engaging, assessing, and formulating sustainable housing strategies for individuals facing homelessness. This role emphasizes intensive case management and crisis intervention, working in partnership with various community organizations to ensure comprehensive service connections and follow-up support for those living on the streets.

KEY RESPONSIBILITIES:

  • Conduct mobile outreach to individuals experiencing homelessness in various locations.
  • Align with the goals of Coordinated Entry System (CES) to minimize street homelessness by connecting participants to essential mental health, housing, and supportive services.
  • Encourage engagement in outreach services through rapport-building and emotional support.
  • Collaborate with mobile medical teams to facilitate access to necessary healthcare services.
  • Work in conjunction with other outreach organizations in the community.
  • Provide crisis prevention and intervention services as required.
  • Deliver ongoing, housing-focused case management using a Harm Reduction and Housing First approach.
  • Ensure completion of referrals and provide advocacy as necessary.
  • Maintain accurate records in both written and electronic formats.
  • Participate in regular meetings as designated by the supervisor.
  • Utilize personal and agency vehicles for client transportation as needed.
  • Perform additional tasks and responsibilities as assigned.

QUALIFICATIONS:

  • Understanding of the challenges faced by program participants, including health and mental health issues.
  • Ability to engage empathetically and without judgment with individuals experiencing homelessness.
  • Flexibility to adapt to a dynamic work environment.
  • Willingness to work after hours for crisis situations.
  • Familiarity with community social service resources.
  • Strong conflict resolution and time management skills.
  • Excellent written and verbal communication abilities.
  • Bilingual skills in Spanish/English are preferred.
  • Experience in collaborative team settings.

ADDITIONAL REQUIREMENTS:

  • Maintain confidentiality in accordance with HIPAA standards.
  • Obtain CPR/First Aid Certification.
  • Pass post-offer Tuberculosis (TB) clearances.
  • Regular travel is required for this position.
  • Possess a valid California Driver's License and maintain auto insurance.
  • Ability to work a 9/80 schedule.

EXPERIENCE:

  • Minimum of one year working with individuals experiencing homelessness or two years in health or behavioral health counseling.
