With over 40 years of experience and a track record of transforming the lives of more than 13,000 individuals annually, we are committed to ending homelessness. ROLE SUMMARY:
The Outreach Specialist will be an integral member of a Street-based Multidisciplinary Team (MDT), responsible for engaging, assessing, and creating permanent housing strategies for individuals facing homelessness. This role will employ best practices to deliver intensive case management and crisis intervention services. Collaboration with various community organizations will be essential to ensure individuals receive the necessary services and follow-up support.
KEY RESPONSIBILITIES:
- Conduct mobile, field-based outreach to individuals experiencing homelessness in various settings.
- Align with the goals of the Coordinated Entry System (CES) to decrease street homelessness by connecting participants with mental health, healthcare, housing, and other supportive services.
- Encourage individuals to engage in outreach services and community referrals that enhance their well-being through rapport-building and emotional support.
- Collaborate closely with mobile medical teams to ensure individuals receive necessary medical care and health assessments.
- Work in partnership with other outreach organizations in the community.
- Provide crisis prevention, intervention, and de-escalation services as required.
- Deliver ongoing street-based case management services focused on housing, utilizing a Harm Reduction and Housing First approach.
- Ensure that referrals are completed and provide advocacy as needed.
- Maintain accurate records in both written and electronic formats.
- Participate in regular staff, agency, and community meetings as directed.
- Travel within the service area using personal and agency vehicles as needed.
- Perform additional tasks and responsibilities as assigned.
QUALIFICATIONS:
- Understanding of the challenges faced by program participants, including health, substance abuse, mental health, and legal issues.
- Ability to engage empathetically and without judgment with individuals experiencing homelessness.
- High tolerance and understanding for individuals with mental and physical health needs.
- Flexibility to adapt to a dynamic work environment.
- Availability for after-hours crisis response.
- Familiarity with community social service agencies and resources.
- Strong conflict resolution and time management skills.
- Excellent written and verbal communication abilities.
- Experience with electronic data entry and documentation.
- Bilingual skills in Spanish/English are preferred.
- Proven ability to work collaboratively within team settings.
ADDITIONAL REQUIREMENTS:
- Maintain confidentiality in accordance with HIPAA standards.
- Obtain and keep current CPR/First Aid Certification.
- Pass post-offer Tuberculosis (TB) clearances.
- Regular travel is required for this position.
- Possess a valid driver's license and maintain auto insurance.
- Ability to work a 9/80 work schedule.
EXPERIENCE:
- At least one year of experience working with individuals experiencing homelessness or two years of experience in health, behavioral health, or substance use counseling.
