Daily Responsibilities:
  • Assess, plan, and implement individualized care strategies for members, ensuring the most appropriate and least restrictive level of care.
  • Identify and initiate referrals for social services, including financial, psychosocial, community, and state supportive services.
  • Manage care plans throughout the continuum of care as a single point of contact.
  • Communicate with stakeholders to ensure quality coordinated care and services are provided to members.
  • Advocate for patients and families to ensure their needs and choices are represented and supported by the healthcare team.
  • Perform telephonic or face-to-face clinical assessments to identify, evaluate, coordinate, and manage member needs.
  • Identify high-risk complications and coordinate care with the member and healthcare team.
  • Provide input and data to direct supervisors related to internal or external mandatory audits or reporting.
Required Qualifications:
  • Clear and active Texas LVN license.
  • 3-5 years of experience in a Managed Care environment working with individuals with chronic illnesses or disabilities.
  • Service Coordinator experience or Star Plus experience.
