Position Overview:

The Residential Support Coordinator serves as the main liaison for individuals who are elderly or face mental and physical health challenges that hinder their participation in programs. This role is crucial in providing case management, daily engagement, and essential social services to each individual in the program. The Coordinator will aid residents in achieving the necessary stability to engage in on-site programs or secure permanent housing off-site.

Key Responsibilities:

  1. Conduct daily assessments and monitor progress for residents within assigned caseload, ensuring the development and execution of personalized case plans. (Document detailed notes immediately following each session.)
  2. Oversee all residents in the program and facilitate daily activities while on duty.
  3. Implement crisis interventions and conflict resolution strategies for residents, utilizing compassionate principles to address their needs.
  4. Ensure residents attend regular appointments to meet their mental and physical health care requirements.
  5. Assist residents in coordinating services and obtaining mainstream benefits such as TANF, Unemployment, SSDI/SSI, Medicaid/Medicare, and other necessary referrals, including application reviews and follow-up communications.
  6. Provide training sessions and classes, both group and individual, to residents as assigned.
  7. Issue client residency letters, conduct room inspections, and manage other administrative tasks.
  8. Transport residents to external appointments and activities as needed.
  9. Participate in regular team meetings with counselors as required.
  10. Refer clients with specific needs to the appropriate counselors.
  11. Attend case management meetings and other relevant gatherings.
  12. Enforce organizational rules, policies, and procedures as necessary.
  13. Screen clients for substance use and provide interventions when needed.
  14. Maintain records of room inspections and other relevant documentation.
  15. Conduct random file reviews to ensure compliance.
  16. Document log entries to maintain continuity of support services.
  17. Engage in training sessions as assigned.
  18. Respond promptly to any unsafe situations, taking responsibility for the well-being of all residents.

Additional Responsibilities:

  1. Engage in ministry and prayer activities as appropriate.
  2. Perform other duties as assigned.

Qualifications:

Education and Experience: A Bachelor's degree in a social services-related field (such as social work, counseling, psychology, sociology, or theology) is required, along with three years of relevant experience or an equivalent combination of education and experience. A Master's degree in Social Work or Counseling is preferred. A minimum of three years of experience in social sciences as a Case Manager or Rehabilitation Specialist is essential. Experience in drug rehabilitation or counseling is a plus. A valid driver's license and a clean driving record are required.

Skills:

Strong reading, writing, time management, and multitasking abilities are essential. Good judgment in handling confidential matters and setting priorities is crucial. The ability to communicate effectively and work collaboratively with various stakeholders is necessary.

Work Environment:

This position typically operates during normal business hours in an office setting, with occasional evening or weekend events. Frequent visits to program sites are required, and the work schedule may vary to meet organizational needs.

Physical Requirements:

This role involves medium work, including lifting up to 20 pounds, frequent sitting, talking, and repetitive motions, as well as occasional standing and walking.
