The Clinical Advanced Practice Provider operates under a collaborative practice agreement to deliver specialized nursing care for patients in acute healthcare environments. Working alongside physicians and other healthcare professionals, the Advanced Practice Provider formulates and organizes care plans aimed at achieving the best possible patient outcomes. As a clinical authority, the provider executes tailored treatment strategies, assesses results, adjusts care plans as necessary, coordinates complex patient care, offers patient education, and aids in discharge planning for patients within their designated service line.

KEY RESPONSIBILITIES:

  • Exhibits the knowledge and competencies necessary to provide age-appropriate care, understanding the physical and psychological needs of the patient population served.
Assessment/Diagnosis/Outcomes Identification
  • Initiates and interprets relevant diagnostic tests and procedures based on the patient's current condition.
  • Utilizes comprehensive data gathered during patient interviews, examinations, and diagnostic evaluations to establish diagnoses.
  • Identifies anticipated outcomes that integrate cost-effectiveness, clinical efficacy, patient satisfaction, and continuity of care among providers.
Planning/Implementation/Evaluation
  • Develops strategies tailored to meet the needs of complex patients, incorporating their values and beliefs into the care plan.
  • Conducts case management responsibilities within the corresponding clinical service to achieve targeted patient care and optimal resource utilization.
  • Recognizes patient and family needs related to the continuity and efficiency of care.
  • Facilitates communication by serving as a liaison among physician consultants, healthcare team members, patients, and families to ensure integrated care throughout the continuum.
  • Prescribes specific pharmacological treatments and interventions based on clinical indicators, patient status, and diagnostic results.
  • Provides ongoing education to patients and families regarding their care plans.
  • Evaluates the effectiveness of interventions and care plans, recommending modifications as necessary.
  • Develops innovative solutions to patient care challenges that enhance resource utilization and maintain quality standards.
Research/Quality Improvement
  • Participates in the development and execution of protocols, order sets, and other clinical improvement initiatives.
  • Encourages evidence-based nursing practices by integrating research findings into clinical practice and promoting the use of educational and managerial research outcomes.
  • Identifies opportunities for process improvement, leads quality initiatives, and implements necessary changes.
  • Actively engages in research and performance improvement activities.
Staff Development/Education
  • Demonstrates expert practice to the healthcare team, serving as a clinical resource for patient care and fostering critical thinking skills among staff.
  • Collaborates with staff educators to prepare and deliver formal continuing education programs for nursing personnel.
  • Maintains clinical expertise through participation in professional organizations, ongoing education, and staying current with relevant literature.
  • Mentors nursing staff, providing informal education and role modeling, while encouraging participation in clinical discussions and care planning.
Professionalism/Leadership
  • Engages in a structured process to seek feedback on personal practice from patients, peers, and professional colleagues.
  • Initiates and updates protocols or guidelines to reflect evidence-based practices and address emerging clinical challenges.
  • Advocates for the dissemination of information and advancement of the nursing profession through writing, publishing, and presenting to both professional and lay audiences.
  • Supports organizational and administrative initiatives to achieve departmental and organizational objectives related to patient safety, clinical excellence, customer satisfaction, employee satisfaction, financial performance, and shared leadership.
  • Coordinates daily patient needs with case managers and social workers as appropriate.
QUALIFICATIONS:

Required Education and/or Experience:
  • Master's Degree in Nursing (MSN or MS)
  • Certification in the relevant specialty area
Preferred Education and/or Experience:
  • Prior experience as an Advanced Practice Provider
Required Licenses:
  • Current Registered Nurse license in the State of Illinois
  • Current Advanced Practice Nurse license in the State of Illinois along with a collaborative practice agreement
A high level of analytical ability is essential, as incumbents must gather, analyze, and interpret data to resolve complex problems requiring professional knowledge in a specific discipline and/or design intricate clinical or administrative systems and programs.

Strong communication skills are necessary to effectively interact with all members of the healthcare team.

Work typically involves activities, projects, or assignments where considerable decision-making authority regarding procedures, plans, and schedules is exercised.
