The Clinical Advanced Practice Specialist operates under a collaborative practice agreement to deliver advanced nursing services for patients in an acute care environment. In partnership with physicians and other healthcare professionals, the specialist formulates and coordinates care strategies to achieve the best possible patient outcomes. As a clinical authority, the specialist executes tailored treatment plans, assesses results, adjusts treatment strategies as necessary, manages care for intricate cases, educates patients, and aids in discharge planning for patients within their designated service line.

PRIMARY DUTIES AND RESPONSIBILITIES: (The following responsibilities are essential job functions, as defined by the ADA, except those that begin with the word "May.")

  • Exhibits the knowledge and skills necessary to provide care appropriate to the age of patients served, including understanding the physical and psychological needs of patients and responding suitably to those needs.
Assessment/Diagnosis/Outcomes Identification
  • Initiates and interprets diagnostic tests and procedures pertinent to the patient's current condition.
  • Utilizes comprehensive data and information gathered during interviews, examinations, and diagnostic tests to identify diagnoses.
  • Recognizes expected outcomes that encompass cost and clinical effectiveness, patient satisfaction, and continuity among providers.
Planning/Implementation/Evaluation
  • Formulates strategies to address the needs of complex patients, integrating their values and beliefs into the care plan.
  • Performs case management responsibilities related to the clinical service, focusing on targeted patient care and efficient resource utilization.
  • Identifies patient and family needs concerning continuity and efficiency of care.
  • Facilitates communication by serving as a liaison among physician consultants, other healthcare team members, the patient, and family to ensure integrated care across the continuum.
  • Prescribes specific pharmacological agents and/or treatments based on clinical indicators, the patient's condition, and diagnostic and laboratory test results.
  • Provides ongoing education to patients and families regarding their care plan.
  • Evaluates the effectiveness of interventions and care plans, recommending modifications as appropriate.
  • Develops innovative solutions for patient care challenges that address effective resource utilization and maintain quality.
Research/Quality
  • Engages in the design and implementation of protocols, order sets, and other clinical improvement tools.
  • Encourages evidence-based practice in nursing by integrating research findings into practice and promoting the use of clinical, educational, and managerial research outcomes.
  • Drives process improvement by identifying opportunities for enhancement, leading quality initiatives, and executing change.
  • Actively participates in research and performance improvement activities.
Staff Development/Education
  • Models expert practice to the healthcare team, serving as a clinical expert/resource for patient care and assisting staff in developing critical thinking skills.
  • In collaboration with staff educators, prepares and conducts formal continuing education programs for nursing personnel.
  • Maintains clinical expertise through participation in professional organizations, ongoing education, and staying current with relevant literature.
  • Mentors nursing staff, providing informal education and role modeling, and encourages participation in rounds, voicing questions and concerns, and articulating care plans.
Professionalism/Leadership
  • Engages in a formal process to seek feedback regarding one’s practice from patients, peers, professional colleagues, and others.
  • Initiates and revises protocols or guidelines to reflect evidence-based practice, accepted changes in care management, or to address emerging issues.
  • Promotes the communication of information and advancement of the profession through writing, publishing, and presentations for both professional and lay audiences.
  • Supports organizational and administrative initiatives to achieve departmental and organizational goals related to patient safety, clinical excellence, customer satisfaction, employee satisfaction, financial performance, and shared leadership.
  • Coordinates daily patient needs with case managers and social workers as appropriate.
KNOWLEDGE, SKILLS AND ABILITIES REQUIRED:

Required Education and/or Experience:
  • Master's Degree in Nursing (MSN or MS)
  • Certification in the area of specialty
Preferred Education and/or Experience:
  • Previous experience as an Advanced Practice Nurse
Required Licenses:
  • Current State of Illinois Registered Nurse license
  • Current State of Illinois Advance Practice Nurse license and a collaborative practice agreement
A relatively high level of analytical ability is required. Incumbents must collect, analyze, and interpret data to solve problems requiring professional knowledge in a specific discipline and/or design complex clinical or administrative systems and programs.

A relatively high level of communication skills is required to effectively engage with all healthcare team members.

Work generally consists of activities, projects, or assignments where there is considerable decision-making authority regarding procedures, plans, and schedules.
