POSITION HIGHLIGHTS:

ROLE OBJECTIVE:


The Registered Nurse (CAP RN 1) position serves as an introductory role within the Clinical Advancement Program for Registered Nurses.

The CAP RN 1 is tasked with delivering and coordinating proficient clinical care, which includes patient assessment, care planning, intervention, and evaluation for a designated group of patients.


As a vital member of the Kidney Disease healthcare team, the CAP RN 1 engages in decision-making, educational initiatives, leadership roles, and quality improvement efforts aimed at enhancing patient care outcomes and operational efficiency.


KEY RESPONSIBILITIES:


All duties are to be performed in alignment with Fresenius Kidney Care policies, procedures, and applicable nursing standards, as well as state and federal regulations.


Under the supervision of the Program Manager (PM) and with support from the Educator, Preceptor, or a more experienced CAP RN, the new hire will perform essential functions for the initial six months of employment.


Completion of requirements to progress to the next Inpatient CAP level within 24 months from the hire date is expected.


Conducts systematic collection and analysis of patient data before, during, and after treatment, documenting findings in the patient medical record, adjusting treatment plans as necessary, and notifying relevant healthcare team members.


Identifies variations in patient status and collaborates with appropriate team members for further input.

Assesses daily patient care needs and collaborates with both direct and ancillary care staff as necessary.

Delivers safe and effective patient care according to prescribed treatment protocols.


Facilitates communication with both Fresenius Kidney Care and external dialysis providers, as well as relevant hospital personnel, to ensure continuity of care.

Administers medications as prescribed, adhering to contractual agreements.

Ensures nephrology physician orders are accurately documented in the Medical Record.

Accurately enters all treatment data into the designated medical record in a timely manner.


Implements, administers, monitors, and documents patient responses to prescribed intradialytic transfusions, including notifying physicians and blood suppliers of any adverse reactions.


Familiarizes with emergency policies and procedures to effectively respond to emergencies.

Delegates appropriate tasks to patient care staff, including LVN/LPNs and Patient Care Technicians.

Monitors patient care staff for compliance with techniques and adherence to policies and procedures.

Promotes infection control and ensures equipment and environmental safety.

Assesses and documents patient and family educational needs to provide ongoing education.

Instructs facility staff on specialized care related to renal replacement and apheresis therapies.

Collaborates with the interdisciplinary team to update nursing assessments and care plans for assigned patients.

Initiates and documents ongoing Quality Assurance and Improvement activities, including monthly reports as directed.

Serves as a resource for the healthcare team and participates in training and orientation for new staff as assigned.

Attends all required staff meetings as scheduled.

May assist in outpatient facilities as needed.

Supports and promotes the mission, vision, values, and customer service philosophy of Fresenius Medical Care North America.

Performs additional duties as assigned by management, adhering to state, federal, and contractual standards.

PHYSICAL REQUIREMENTS AND WORK ENVIRONMENT:

Areas of practice include hemodialysis, peritoneal dialysis, continuous renal replacement therapies, and apheresis.

This position involves direct patient care, which may require heavy lifting and assistance with patient mobility. Equipment aids and/or coworkers may assist as needed. Frequent, prolonged periods of standing are required, and the employee must be able to bend over.

Occasional movement of equipment weighing up to 200 lbs. and lifting of chemical and water solutions up to 30 lbs. may be necessary.

The work environment is typical of a healthcare facility, with controlled air temperature and moderate noise levels. Exposure to infectious and contagious diseases/materials may occur.


Essential functions may be required at various locations with similar physical demands and working conditions.

Daily responsibilities include desk work, computer tasks, and interaction with patients, facility staff, and physicians.

Travel between assigned facilities and community locations may be required, as well as attendance at regional meetings.

Participation in on-call rotation is necessary (Night, Weekend, or as defined by program needs).

SUPERVISION:
Oversight of Patient Care Technicians/LPNs may be assigned as a designated Nurse in charge, following:

Successful completion of all required education and training, plus

A minimum of 9 months of RN experience, along with

6 months of dialysis experience.

EDUCATION:
Graduate of an accredited School of Nursing.

Current state licensure is required.

Must meet practice requirements in all states of employment.

EXPERIENCE AND REQUIRED SKILLS:


This position is entry-level for RNs with less than 2 years of nephrology nursing experience within the last two years or for RNs with a BSN and less than 1 year of nephrology nursing experience within the last two years.

A minimum of 9 months of experience as a Registered Nurse is preferred.

At least 6 months of nephrology experience is required before independent work without a qualified dialysis RN.

6 months of dialysis experience is necessary to be on-call.

Hemodialysis and/or ICU experience is preferred.

Successful completion of all required education and training for new employees is mandatory.

CPR BLS Certification must be maintained.

All employment conditions, including color vision testing, must be met.

Completion of all annual required education/training/competencies is essential.

Ongoing training and education as assigned or required must be completed.
