Position Overview

Role Responsibilities

The Care Manager (RN) plays a crucial role in assisting members eligible for care coordination and case management services to achieve their highest level of health.

This position is responsible for engaging with members and healthcare providers to evaluate, plan, and set personalized goals for each member. The Care Manager will facilitate and coordinate care while ensuring quality and the efficient use of resources.

Acting as a primary point of contact, the Care Manager advocates for members within the care coordination program.

Key duties include:

  • Assessing members to identify care coordination and case management needs.
  • Conducting comprehensive evaluations of environmental, psychosocial, and support requirements.
  • Identifying barriers to care and implementing suitable care management strategies.
  • Developing individualized care plans to help members overcome challenges and achieve their health objectives.
  • Setting both short-term and long-term goals with defined timelines for completion.
  • Collaborating with members and their families to communicate goals effectively.
  • Determining the appropriate level of intervention based on member needs and clinical progress.
  • Scheduling follow-up communications as needed and making necessary referrals.
  • Documenting progress towards goal attainment and problem resolution.
  • Coordinating care and services with various stakeholders, including Account Managers, Complex RN Case Managers, and healthcare providers.
