Patient Care Coordinator - (Georgetown)
Accountable for the orchestration of patient services throughout the spectrum of Radiation Oncology care for all individuals. Provides education, disseminates information, supports patients, and facilitates treatment strategies. Acts as a patient advocate, guiding patients and their families through the cancer care and survivorship journey.

Education:

Bachelor's degree in nursing (BSN) from an accredited academic nursing program is required.
Master's degree is preferred.
Experience:

3-4 years of clinical nursing experience is required.
Experience in surgical oncology, inpatient/outpatient/community oncology services is preferred.
Knowledge, Skills, and Abilities:


Must possess clinical maturity and experience, exceptional interpersonal skills, and the ability to work collaboratively with a multidisciplinary team of healthcare providers.

Effective oral and written communication skills are essential.
This role requires a high level of advocacy and problem-solving capabilities.

Ability to manage multiple tasks and organize advocacy and support for a diverse group of cancer patients and their families.

Familiarity with community resources is advantageous.

Primary Duties and Responsibilities:
Conducts the intake, assessment/triage, coordination, and navigation of all patients referred for Radiation Therapy.
Serves as a liaison for referring physicians seeking information or access to the healthcare system.
Evaluates patients for appropriate information and manages the acquisition of relevant medical records for initial consultations.
Responsible for planning and executing the patient evaluation process.
Facilitates patient evaluations by collaborating with referring physicians and MedStar services, including:

Radiology, Medical Oncology, Radiation Medicine, and other MedStar services as necessary.

Coordinates the intricate management of patients at various stages of their diagnosis, treatment, and follow-up, working with all disciplines involved in the patient's care to ensure timely progress toward optimal clinical outcomes in the most efficient manner.

Identifies opportunities for patient participation in clinical trials. Communicates information to physicians, research coordinators, and referring physicians to encourage clinical trial enrollment. Responsible for clinical trial Informed Consent and specimen collection as required.

Navigates the patient/caregiver through the continuum of care, providing education and resources regarding medical conditions, diagnostic procedures, treatments, clinical trials, support services, hospital processes, and other pertinent information.

Serves as a continuous resource for patient/caregiver education and support.
Communicates patient progress/disposition to healthcare providers.
Assists in maintaining outcome databases.
Supports the management of IRB communications and regulatory updates.
Possesses a working knowledge and access to multiple electronic systems to facilitate patient care and documentation.
Participates in Oncology Tumor Board meetings.
Collaborates closely with physicians and the marketing department to develop promotional ideas and materials for the Radiation Therapy Program.
Identifies needs and creates patient education and support materials.
Engages in community education and outreach.
Assumes additional duties and responsibilities that are appropriate to the position and area.

This position has a hiring range of $87, $149,094.40
