Position Overview:
We are seeking a full-time Care Coordinator I for our medical clinic.

The Care Coordinator I plays a vital role in managing care for patients with chronic conditions and behavioral health needs directly at the point of care.

As an integral member of the Clinical Care Team, the Care Coordinator will engage in daily meetings, assess patient needs following established protocols, and deliver care services.

This role is essential for enhancing the quality and patient-centered approach at Sea Mar Community Health Centers.


Key Responsibilities:
  • Participate in daily meetings to anticipate and address patients' clinical, social, and behavioral health needs.
  • Collaborate with the care team to identify and resolve gaps in care using process improvement techniques.
  • Advocate for patient services with community resources, social services, and healthcare providers.
  • Facilitate care transitions for patients discharged from emergency rooms or hospitals.
  • Monitor patient adherence to care plans and communicate findings to primary care providers.
  • Organize and lead monthly Health Home meetings, collaborating with clinic management to create agendas and facilitate discussions.
  • Work with the clinical team to enhance Patient-Centered Medical Home processes and document performance metrics.
  • Review medical records for quality assurance and utilization indicators as per the Quality Improvement Plan.
  • Train new staff on the Chronic Care Model and Patient-Centered Medical Home principles.
  • Perform additional duties as assigned.

  • Strong interpersonal and communication skills, with the ability to establish therapeutic relationships with patients.

Knowledge, Skills, and Abilities:

Preferred qualifications include:

  • Understanding of the Patient-Centered Medical Home Model and motivational interviewing techniques.
  • Knowledge of evidence-based care standards for chronic and behavioral health conditions.
  • Proficiency in Microsoft Office Suite (Word, Excel, PowerPoint, Outlook).
  • Ability to document patient information accurately in Electronic Health Records.
  • Familiarity with community resources.
  • Capacity to thrive in a fast-paced community health environment.
  • Analytical thinking and problem-solving skills within a multidisciplinary team.
  • Effective communication with diverse populations.
  • Time management and task prioritization skills.
  • Ability to assess client learning needs and readiness for education.
  • Commitment to maintaining confidentiality and adhering to privacy regulations.
  • Excellent customer service and communication skills.
  • Critical thinking and process improvement implementation skills.
  • Bilingual in Spanish is highly preferred; other language skills may be considered based on community needs.

Education and Experience:

Applicants should possess:

  • Medical Assistant Training with at least one year of experience in a community health or family practice setting, or equivalent experience.
  • Current CPR certification within 90 days of hire, with ongoing maintenance required.
  • Valid Washington State Driver's License and proof of liability insurance.
