Position Overview:
The Care Coordinator I position is a full-time role based at our medical clinic.

This role is pivotal in managing care for patients with chronic health conditions and behavioral health requirements at the point of service.

As part of the Clinical Care Team, the Care Coordinator will engage in daily meetings, assess patient needs following established protocols, and deliver care services directly.

The Care Coordinator plays a crucial role in enhancing the quality of patient-centered care at Sea Mar Community Health Centers.


Key Responsibilities:
  • Participate in daily meetings to anticipate and address patients' clinical, social, and behavioral health needs.
  • Collaborate with the care team to identify and address gaps in care using process improvement techniques.
  • Advocate for patient services with community organizations, social services, and healthcare providers.
  • Facilitate care transitions for patients discharged from emergency rooms or hospitals.
  • Monitor patient adherence to care plans and communicate outcomes to primary care providers.
  • Organize and lead monthly Health Home meetings, coordinating with clinic management.
  • Work with the clinical team to enhance Patient-Centered Medical Home processes and document performance metrics.
  • Review medical records for quality and utilization indicators as per the Quality Improvement Plan.
  • Train new staff on the Chronic Care Model and Patient-Centered Medical Home principles.
  • Perform additional duties as assigned.

Knowledge, Skills, and Abilities:

Preferred candidates will possess:

  • Understanding of the Patient-Centered Medical Home Model and motivational interviewing techniques.
  • Knowledge of evidence-based care standards for chronic and behavioral health conditions.
  • Proficiency in Microsoft Office Suite and Electronic Health Records.
  • Ability to analyze patient care data and assess client learning needs.
  • Excellent communication and customer service skills.
  • Critical thinking and problem-solving abilities.
  • Bilingual skills in Spanish are strongly preferred.

Education and Experience:

Successful candidates will have:

  • Medical Assistant Training with at least one year of experience in a community health or family practice setting, or equivalent experience.
  • Current CPR certification within 90 days of hire, with ongoing maintenance required.
  • A valid driver's license and proof of liability insurance.
