About the Role

The RN Case Manager - Care Coordination Specialist will collaborate with the interdisciplinary care team to develop, implement, and maintain comprehensive care plans for patients. This role will facilitate an interdisciplinary approach to patient care and discharge planning, ensuring seamless transitions from hospital to community settings.

Key Responsibilities
  • Collaborate with the care team to identify patients with discharge needs and develop individualized care plans
  • Facilitate communication between healthcare providers, patients, and families to ensure continuity of care
  • Coordinate with community resources and healthcare providers to ensure timely and effective discharge planning
  • Monitor and evaluate the effectiveness of care plans, making adjustments as needed
  • Participate in quality improvement initiatives to enhance patient outcomes and care coordination processes
Requirements
  • Bachelor's degree in Nursing from an accredited institution
  • 2 years of hospital experience in general Medical/Surgical practice
  • Registered Nurse licensure in the State of Wisconsin
  • Knowledge of discharge planning principles, including disease processes and community resources
  • Proficient computer skills and ability to navigate electronic health records
  • Strong communication and interpersonal skills, with ability to work effectively in a team environment
Preferred Qualifications
  • 1-2 years of experience in home care or case management
  • Knowledge of healthcare environment and payer-based rules
  • Certification in Case Management
