Job Summary

We are seeking a skilled and compassionate RN Case Manager to join our homecare team. As a RN Case Manager, you will work collaboratively with our interdisciplinary team to provide patient-centered care and support to individuals and families in their homes.

Key Responsibilities
  • Conduct comprehensive patient assessments to identify physical, emotional, and spiritual needs
  • Develop and implement individualized plans of care to meet patient needs and goals
  • Provide education and support to patients and families on pain management, symptom control, and end-of-life care
  • Collaborate with the interdisciplinary team to ensure seamless care coordination and communication
  • Document patient progress and outcomes in our electronic health record system
Requirements
  • ADN or BSN degree required
  • Current, unrestricted Registered Nursing License in the state of employment
  • One year of nursing experience preferred
  • Valid driver's license, auto insurance, and access to a reliable vehicle
  • Ability to develop and maintain knowledge of hospice and end-of-life care
  • Ability to maintain professional boundaries and confidentiality
  • Basic Microsoft Office skills and intermediate electronic health records skills
