Job Description

Job Summary

We are seeking a highly skilled and compassionate Home Health RN Case Manager to join our team at Oakwood Village Univ Woods Homes, Inc. As a key member of our interdisciplinary team, you will be responsible for providing comprehensive care to our geriatric clients in their homes.

Key Responsibilities

  • Assessment and Planning: Conduct thorough assessments of clients' health needs and develop individualized plans of care to meet their rehabilitative, therapeutic, and preventative needs.
  • Care Coordination: Coordinate medical orders, communicate with healthcare providers, and ensure seamless transitions of care.
  • Education and Support: Educate clients and their families on disease management, medication adherence, and healthy lifestyle choices.
  • Discharge Planning: Develop and implement discharge plans to ensure clients receive the necessary support and resources for a smooth transition to community-based care.
  • Collaboration and Communication: Work collaboratively with the interdisciplinary team, including physicians, therapists, and social workers, to ensure comprehensive care.
  • Leadership and Professional Development: Participate in continuing education programs, case conferences, and quality improvement initiatives to enhance clinical skills and stay up-to-date on industry best practices.

Requirements

  • Education and Experience: Graduate of an accredited school of nursing with a current RN license in the State of Wisconsin. At least three to five years of experience in home health care, preferably with geriatric clients.
  • Skills and Qualifications: Strong assessment and problem-solving skills, excellent communication and interpersonal skills, and ability to work independently and as part of a team.
