Job Summary

As a RN Case Manager at SSM Health, you will play a critical role in ensuring the seamless transition of patients from hospital care to their next level of care. This position requires a skilled and compassionate nurse who can assess, develop, and implement effective discharge plans that meet the unique needs of each patient.

Key Responsibilities
  • Assess patients' discharge planning needs and develop individualized plans to ensure a safe and successful transition to their next level of care.
  • Collaborate with the interdisciplinary team to identify and address any barriers or gaps in care that may impact a patient's discharge.
  • Develop and implement strategies to reduce readmissions and improve patient outcomes.
  • Participate in quality improvement initiatives to enhance the discharge planning process and patient care.
  • Communicate effectively with patients, families, and healthcare providers to ensure a smooth transition of care.
Requirements
  • Graduate of an accredited school of nursing with a current RN license in the state of employment.
  • Three years of acute care experience, preferably in a case management or discharge planning role.
  • Strong assessment and critical thinking skills, with the ability to analyze complex patient data and develop effective solutions.
  • Excellent communication and interpersonal skills, with the ability to work effectively with patients, families, and healthcare providers.
  • Ability to work in a fast-paced environment and prioritize multiple tasks and responsibilities.
