POSITION SUMMARY

The Support Specialist for Community Wellness plays a crucial role in empowering clients to lead fulfilling lives. This position involves assisting individuals in developing essential skills for independent living, pursuing education and training, securing employment, and engaging in meaningful social interactions. The Support Specialist will facilitate clients' transitions to autonomy and advocate for the utilization of community resources. Furthermore, this role includes coordinating care related to clients' health, housing, and vocational aspirations.

Responsibilities Include:

  • Conducting essential community support and skill-building activities as outlined in client care plans.
  • Identifying necessary resources and assisting clients in accessing them.
  • Providing individualized interventions to develop interpersonal and community coping skills.
  • Supporting clients during crisis situations.
  • Engaging clients to enhance their participation in treatment.
  • Delivering services in natural environments, including homes and community settings.
  • Making referrals to external agencies as needed.
  • Educating and consulting with clients' families to promote their well-being.
  • Assisting clients in monitoring and managing their symptoms to improve daily functioning.
  • Communicating with clinical management regarding client compliance with service goals.

EDUCATIONAL REQUIREMENTS:

  • High School Diploma or equivalent.
  • Demonstrated experience in providing support services.
  • Strong communication and interpersonal skills.
  • Commitment to a client-centered approach.
  • Valid driver's license and access to transportation.
