Core Responsibilities:

As a Care Manager, your primary responsibilities will include:

  • Empowering youth and families to leverage informal supports and sustainable community resources for a successful transition to independence.
  • Collaborating with internal and external partners, including educational institutions, healthcare providers, judicial systems, and treatment organizations, to ensure effective Wraparound care management.
  • Ensuring health and safety by adhering to Agency, State, and Medicaid policies, procedures, standards, and reporting protocols within designated timeframes.
  • Exhibiting professionalism and flexibility to meet the needs of youth, families, and the Agency in various settings, including home, community, and telehealth.
  • Promoting talent engagement and development by acting as an ambassador for your team, the families served, and the agency.

Qualifications & Skills:

Essential Qualifications and Skills for Applicants:

  • Bachelor's Degree in social work, counseling, psychology, or a related discipline.
  • A minimum of one year of relevant experience.
  • Experience in crisis management, youth with substance use disorders, and/or intellectual and developmental disabilities is advantageous.
  • Bilingual language skills are a plus.
  • Basic proficiency in technology.
  • Exceptional time management and organizational abilities.
  • Culturally informed approach.
  • Possession of a valid driver's license and access to a personal vehicle.
