Role Responsibilities:

  • Implement the Wraparound model in all interactions with families and children.
  • Supervise Care Managers in their collaboration with families to establish a Child Family Team, which includes:
    • The youth and their parent/caregiver
    • Natural supports chosen by the family
    • A representative from the Family Service Organization (FSO) if desired
    • A clinical staff member involved in the youth's treatment, if requested
    • Representatives from external agencies relevant to the youth's situation
  • Coordinate with the Family Service Organization (FSO) to provide caregiver support.
  • Ensure timely Child Family Team Meetings.
  • Oversee quality assurance for service providers regarding the Individual Service Plan and compliance with regulatory standards.
  • Evaluate staff performance and implement improvement plans as necessary.
  • Guide Care Managers in securing and maintaining Medicaid coverage for families.
  • Ensure adherence to Medicaid regulations and standards.
  • Verify that documentation is completed accurately and timely in both electronic and hard copy formats.
  • Monitor the timely completion of Family Crisis Plans by Care Managers.
  • Review and approve Individual Service Plans for thoroughness and accuracy.
  • Ensure compliance with critical incident reporting protocols.
  • Provide supervisory support to on-call staff for youth and families involved with the agency.
  • Maintain professional conduct and uphold the Agency Code of Ethics.
  • Adhere to HIPAA standards to ensure client confidentiality.

Qualifications:

  • Master's Degree in social work, counseling, psychology, or a related field.
  • Relevant licensure (LSW, LCSW, LAC, LPC).
  • At least one year of supervisory experience with direct reports.
  • Experience with individuals, particularly children, facing substance use disorders or developmental disabilities is advantageous.
  • Bilingual skills are a plus.
  • Strong time management and organizational abilities.
  • Culturally competent communication skills, both verbal and written.
  • Basic proficiency in technology.
  • A valid driver's license is required.
