Responsibilities:

  • Perform home visits for assigned patients and families, providing education and support to promote optimal health outcomes.
  • Collaborate with our RNs and advanced practice RNs to develop and implement individualized care plans that meet the unique needs of each patient.
  • Engage patients and caregivers in discussions about their goals of care, advance directives, and treatment plans.
  • Participate in direct patient care, including administering medications and providing wound care, as needed.
  • Advocate for patients and families, ensuring that their needs are met and that care is coordinated and consistent with their goals.
  • Educate patients and families on the trajectory of their illness and the use of medications.
  • Consult with RNs and advanced practice RNs as needed, seeking guidance and support to ensure high-quality care.
  • Comply with policies, procedures, and regulatory mandates.

Qualifications:

  • Graduate of an accredited licensed practical nursing program or accredited school of vocational nursing.
  • Current, unencumbered license to practice as a licensed practical nurse specific to the state you are assigned to work.
  • Valid driver's license, auto insurance, and reliable transportation.
  • Current CPR certification.
  • Two years of experience as an LPN/LVN in a clinical setting, preferably in a home health, palliative, or hospice setting.
