Job Summary:

Evergreen Treatment Services is seeking a highly motivated and compassionate Intake and Engagement Coordinator to join our Behavioral Health team. As a key member of our team, you will play a critical role in helping individuals navigate our services and achieve their goals.

Key Responsibilities:
  • Assist with the intake process, ensuring a smooth transition into services and maintaining high levels of participation during the first 90 days of care.
  • Collaborate with referral sources and internal teams to link individuals to the appropriate services, including case management, counseling, medical services, and other support.
  • Provide ongoing engagement and support to participants, including regular check-ins and monitoring to ensure sustained participation.
  • Help identify potential obstacles to service engagement and work with internal teams to find solutions.
  • Act as a liaison between individuals and service providers within the organization, ensuring timely access to services and addressing any logistical challenges.
Requirements:
  • Associate degree in social work, human services, psychology, or a related field.
  • Minimum of 1-2 years of experience in case management, social services, intake coordination, or a similar role.
  • Familiarity with intake procedures and participant engagement strategies.
  • Active, restriction-free Washington State Driver's License required, with an acceptable driving record.
