Post-Transplant Social Work Specialist

The Hume-Lee Transplant Center at VCU Health is seeking a skilled Clinical Social Worker to join our team in providing exceptional care to patients post-kidney transplant. As a vital member of our interdisciplinary team, you will be responsible for assessing and intervening with social drivers of health, ensuring timely documentation submission to CMS regarding dialysis and transplant treatments, coordinating complex case management services in the community, and supporting adjustment post-transplant.
This role requires someone who can work effectively in a fast-paced environment and has experience working with patients with complex chronic illnesses. You will have the opportunity to collaborate with the healthcare team to provide comprehensive care to patients and their families, promoting optimal health outcomes and improving quality of life.
The ideal candidate will possess excellent communication and interpersonal skills, as well as the ability to work independently and as part of a team. A Master's degree in Social Work from a Council on Social Work Education accredited program and licensure as a Clinical Social Worker in Virginia are required. Experience in acute care settings and knowledge of renal transplant processes are preferred but not necessary.
