Senior Transplant Care Coordinator

VCU Health is seeking a skilled Senior Transplant Care Coordinator to join our team.
About the RoleThis is a unique opportunity to work in a fast-paced environment where you will be responsible for delivering high-quality patient care and coordinating transplant services. As a Senior Transplant Care Coordinator, you will be working closely with a multidisciplinary team to provide comprehensive care to patients and their families.
The ideal candidate will have a strong background in nursing, with at least 3 years of experience in transplant, critical care or medical-surgical nursing. They will also have excellent communication and leadership skills, with the ability to prioritize tasks and manage multiple priorities in a fast-paced environment.
Responsibilities- Deliver direct age-specific care to patients based on medical orders and an integrated plan of care
- Accurately document patient information and communicate with healthcare teams
- Assist in the coordination of transplant services and ensure seamless communication between teams
- Provide education and support to patients and their families throughout the transplant process
To be successful in this role, you will need:
- A Baccalaureate Degree in Nursing from an accredited School of Nursing
- Current RN licensure in Virginia or eligibility
- A minimum of 3 years of nursing experience in transplant, critical care or medical-surgical nursing
- Excellent communication and leadership skills
The estimated salary for this role is $80,000 - $110,000 per year, depending on experience.
