Position Overview
A Patient Care Navigator (PCN) is essential at Mary's Center, serving as the primary point of contact for patients and visitors, ensuring exceptional customer service throughout the clinic and organization.

Reporting Structure
The Patient Care Navigator will report directly to the Patient Care Navigator Manager at their assigned location.

Key Responsibilities
To excel in this role, the individual must effectively carry out the following duties:
  • Welcome patients, visitors, and other guests to Mary's Center.
  • Foster a welcoming and efficient office atmosphere through professional conduct.
  • Relay important information to patients and clinic staff.
  • Comply with all confidentiality and HIPAA regulations.
  • Collaborate with the clinical team to enhance quality and health outcomes.
  • Coordinate patient care and services.
  • Work with external healthcare providers, specialists, and hospital services.
  • Assist in enrolling patients into the care team.
  • Gather socio-demographic data for population health management.
  • Engage in patient education initiatives.
  • Collect and monitor data essential for patient care and health outcome assessments.
  • Implement clinical surveys and questionnaires in preparation for patient care.
  • Evaluate care affordability and assist with insurance or discount program enrollment.
  • Organize and maintain patient records for clinical and public health purposes.
  • Transfer public health information to state health departments regarding reportable infections and vaccinations.
  • Facilitate the exchange of clinical information with other healthcare providers through regional health information systems.
  • Prepare and uphold the patient care area.
  • Perform additional tasks as assigned by supervisors.

Qualifications
The following qualifications are necessary for this role. Reasonable accommodations may be made for individuals with disabilities to perform essential functions:
  • Education: Minimum of a High School Diploma or GED equivalent.
  • Experience: Previous experience in a medical office setting as a receptionist is preferred.

Physical Requirements
Regularly required to sit; frequently required to reach with hands and arms, walk, stoop, kneel, crouch, talk, or hear; must be able to lift objects weighing up to twenty-five (25) pounds.

Work Environment
Primarily in a standard front office setting with a quiet to moderate noise level. May experience exposure to extreme temperatures and humidity due to outdoor conditions.
