Position Overview


The role of a Patient Care Navigator (PCN) is vital at Mary's Center, serving as the initial point of contact for patients and the broader organization.

Bilingual candidates are highly preferred.

Reporting Structure

The Patient Care Navigator will report directly to the Patient Care Navigator Manager at their assigned location.

Key Responsibilities

To succeed in this role, an individual must effectively perform each essential duty, including but not limited to:
  • Welcoming patients, visitors, and others to Mary's Center.
  • Creating a positive and efficient office atmosphere through professional conduct.
  • Communicating important information to patients and clinic team members.
  • Adhering to all confidentiality and HIPAA regulations set by Mary's Center.
  • Collaborating with the clinical team to enhance quality and health outcomes.
  • Coordinating care and services for patients.
  • Facilitating communication with external clinicians, specialists, and hospital services.
  • Enrolling patients into the care team.
  • Gathering socio-demographic data for population health management.
  • Participating in patient education initiatives.
  • Collecting and monitoring data necessary for patient care and health outcomes reporting.
  • Implementing clinical questionnaires and surveys in preparation for patient care.
  • Assessing care affordability and assisting with enrollment in insurance or discount programs.
  • Maintaining, organizing, and coordinating patient records for clinical care and public health.
  • Transferring public health information with state health departments, including reportable infections and vaccinations.
  • Exchanging clinical care information with other providers through regional health information exchanges.
  • Preparing and maintaining the patient care area.
  • Performing other duties as assigned by supervisors.

Qualifications

The qualifications listed below represent the knowledge, skills, and abilities required for this position. Reasonable accommodations may be made to enable individuals with disabilities to perform essential functions.

Education and Experience: A minimum of a High School Diploma or GED equivalent is required. Previous experience as a receptionist in a medical office setting is preferred.


Physical Requirements: Regularly required to sit; frequently required to reach with hands and arms, walk, stoop, kneel, crouch, talk, or hear; must be able to lift objects up to twenty-five (25) pounds.

Work Environment: Primarily in a typical front office setting with a quiet to moderate noise level. May encounter extreme cold, heat, and humidity due to outside weather conditions.
